Employee 360-Degree Review Questionnaire Template (Constructive & Fair)

In modern organizations, annual top-down performance reviews are dying. An employee's direct manager sees only a fraction of their daily collaborative contributions.

To get a holistic evaluation of leadership, execution, and cultural impact, high-performing organizations use 360-Degree Reviews: gathering confidential feedback from peers, direct reports, cross-functional partners, and managers simultaneously.

However, poorly structured 360-degree reviews can cause severe organizational damage:

  • They can turn into toxic popularity contests.
  • Or they become anodyne praise parades where everyone gives everyone top marks out of fear of damaging relationships.

To generate constructive, actionable feedback that accelerates professional development, you need a Competency-Anchored 360 Questionnaire.

Here is the operational template for constructive and fair 360-degree reviews.

The 4 Competency Dimensions

Dimension 1: Execution & Technical Ownership

             ▼
Dimension 2: Collaboration & Cross-Functional Empathy
             │
             ▼
Dimension 3: Problem Solving & Strategic Decision-Making
             │
             ▼
Dimension 4: The Start / Stop / Continue Qualitative Matrix

Dimension 1: Execution & Reliability

"To what extent does this colleague deliver high-quality work on time and take ownership of commitments?"

  • Scale: 1 to 5 (Rarely / Inconsistently / Consistently / Frequently Exceeds / Role Model)
  • Behavioral Anchor: "Does this person follow through on commitments without requiring micromanagement?"

Dimension 2: Cross-Functional Collaboration

"How effectively does this person communicate across team boundaries and handle constructive feedback?"

  • Scale: 1 to 5
  • Behavioral Anchor: "Are they easy to partner with during high-stress project deadlines?"

Dimension 3: Leadership & Initiative

"When facing ambiguity or unexpected blockers, how effectively does this person propose solutions and guide others?"

  • Scale: 1 to 5

The Core Qualitative Matrix: Start, Stop, Continue

Rating numbers only tell half the story. The most impactful section of any 360 review is the Start / Stop / Continue Framework:

1. START (New Growth Opportunities)

"What is one behavior, practice, or responsibility that this person should START doing to increase their impact?"

  • Example: "Start speaking up earlier in architectural planning sessions; your insights are valuable."

2. STOP (Friction Removal)

"What is one behavior or habit that this person should STOP doing because it creates friction or slows down the team?"

  • Example: "Stop jumping into pull requests before the primary reviewer has finished their review pass."

3. CONTINUE (Superpowers)

"What is this person's greatest superpower that they should CONTINUE doing?"

  • Example: "Continue writing crystal-clear incident post-mortems; it sets the standard for the entire engineering organization."

3 Rules for Fair 360 Administration

  1. Strict Reviewer Anonymity: Direct report and peer feedback must always be aggregated and anonymized before being shared with the employee.
  2. Train Reviewers on Constructive Framing: Prohibit vague attacks ("Alex is bad at communication"). Require concrete behavioral examples ("Alex often misses Slack pings during release deployments").
  3. Separate Feedback from Immediate Compensation: Never tie 360 feedback directly to end-of-year bonus payouts on the same day. Keep 360s focused on growth and professional development.

Build high-trust feedback loops, empower your team to grow, and foster a culture of continuous improvement.
